Sand Springs Family Dentistry would like to award a $500 scholarship to a Class of 2021 Senior for their community involvement, volunteer work and/or academic accomplishments.
Here’s how it works:
Nominate a deserving senior by submitting their name, school, parent/guardian contact information and a short explanation as to why this senior has gone above and beyond to make a positive difference or the hardwork he/she has given in the classroom.
The nominator nor the student have to be a patient of our office! The Senior can be a graduate of an area high school Nominations are to be received by May 3rd and the winner will be announced on May 7th Nominations are to be sent via email to: info.sandspringsdds@gmail.com or by FB message
